한국인 하악관의 형태 및 치밀뼈 해면뼈의 구조

Abstract
[한글]인공치아를 만들거나 틀니의 지지구조륨 위해 티타늄 (titanium) 같은 금속재료를 심을 때 하악골 치밀려 (cortical bone)와 해면뼈 (cancellous bone)의 구성 및 하마골의 내부구조에 관한 이해는 필수적으로 요구되며 더욱이 하악 어금니 부위는 이런 구조들 이외 에도 하악관 (mandibular canal)이 위치하고 있어 임상적으로 중요한 의미가 있다. 또한 터끝부위에 성형술을 하는 경우 하악관의 앞쪽 경계 및 터끝구멍 (mental foramen)과 하악관의 위치 관계도 임상적으로 관심의 대상이 되고있다. 이애 이 연구에서는 한국인 어

른의 하악골 마른뼈 93 예와 반쪽의 하악골 52 쪽을 대상으로 방사선사진을 찍고 하악골을 절단하여 이를 영상분석기로 계측, 분석하여 다음과 같은 결과를 얻었다.

1. 방사선사진상에서 하악관이 지나가는 유형은 하악어금니의 뿌리끝에 가까이 위치해서 "높게" 지나가는 I 형이 6.5 %, 하악골 아래모서리에 붙어 "낮게" 지나가는 Ⅱ 형이 18.8 %, 하악골의 중간을 지나는 Ⅲ 형이 61.7 %로 가장 많았으며 하악관이 나타나지 않은 경우는 13.0 %였다.

2. 방사선사진에서 하악관과 턱끌구멍이 나타나는 형태를 분류한 결과, 하악관이 턱끝구멍으로 계속적으로 이행되는 연속형이 28.3 %, 하악관과 턱끝구멍이 명확하게 분리되어 턱끝구멍이 명확히 나타나는 분리형이 21.7 %, 터끝구멍이 하타관과 분리되어 나타나고 턱끝구멍의 경계가 불명확한 퍼진형이 22.4 %였고 턱끝구멍이 보이지 않은 유형이 27.6 % 였다.

3. 얼굴과 혀쪽 관계애서 하마관은 하악가지와 몸통의 혀쪽 치밀뼈판에 바로 가깝개 붙어 앞으로 달리다가 하악 첫째큰어금니의 앞 쪽이나 하마 둘째작은어금니 부위에서 뺨쪽으로 급격히 방향을 바꾸어 터끝구멍으로 열리는 경우가 각각 35.0%로 가장 많았다.

4. 위-아래 관계에서 하악관은 하악구멍에서 비스듬히 아래, 앞 쪽으로 내려 오다가 하악 들째큰어금니 부위에서 각도를 이루고 앞으로 계속되는 경우가 39.0 %로 가장 많았고, 사랑니 부위에서 각도를 이루는 경우가 24.4 %였다.

5. 하악관은 턱끝구멍으로 열리며 터끝구멍의 위치는 하악 둘째작은어금니 아래에 위치한 경우가 가장 많았으며 (66.5 %), 그 중 들째작은어금니 중간 아래에 위치한 예는 27.4 %, 들째작은어금니 앞 쪽 아래에 위치한 예는 22.6 %, 뒤 쪽 아래에 외치한 예는 16.4 %

였다.

6. 하악골 단면 표본에서 턱끝구멍 아래모서리부터 하악관의 위모서리까지 거리는 2.4 ± 0.7 mm였다.

7. 하악골 각 단면에서 해면뼈가 차지하는 비율은 하악가지 오름부위에서 가장크고 앞니 쪽으로 갈수록 작아졌다.

8. 하악골 단면 표본에서 치밀벼의 평균 두께를 계측한 결과, 얼굴쭉 치밀뼈판의 두께는 하악가지 쪽으로 갈수록 두꺼워지고 혀쪽 치밀벼판은 앞니 쪽에서 더 두꺼운 양상을 보였다. 한편 아레모서리 치밀뼈판의 두께는 비슷하였지만 앞니 부위에서 가장 두꺼웠다.

이상의 결과를 미루어 보아 한국인의 하마관은 혀쪽 치밀뼈판을 따라 하악골의 중간을 지나가며 하악관은 얇은 치밀벼로 들러싸여 있어 하악 어금니부위는 임프란트를 박기에 퐁은 위치로 생각된다. 그러나 하악관의 앞쪽 경계는 대부분이 작은어금니 부위까지 뻗쳐

있고 터끝구멍은 들째작은어금니 부위에서 열리기 때문에 터끌부위의 성형수술시 뼈절개는 최소한 하악관의 앞쪽 경계인 송곳니 부위보다 앞에서 이루어져야 할 것으로 판단된다.





[영문]When materials like titanium are implanted to the jaw to provide artificial teeth and to alive the supporting structure for the denture, the mandibular posterior area is the legion to avoid from iajury because of the presence of the mandibular canal. Also, genioplasty being performed on patients, the position of the anterior limit of the mandibular canal and the geometrical relation between the mental foramen and the mandibular canal should be considered on the basic of clinical and anatomical aspects. In an attempt to analyze the intramandibular courses of the mandibuler canal in mandibles and the internal structure of the mandibles, ninety-three adult mandibles in Korean were radiographed and fifty-two hemisected

mandibles were sectionde to observe the intramandibular courses of the mandibular canal and the mental foramen, and to evaluate the thickness of the mandibular cortical plates on image analyzer. Results are as fort lowings.

1. On radiographs, the intramandibular courses of the mandibular canals were classified into four types: type I in which the single canal located approximately to the mandibular teeth running "high" within the mandible, type Ⅱ in which the

single canal located approximately to the mandibular inferior border running "low" within the mandible, type Ⅲ in which located in the "middle" portion of the mandible, type IV in which "invisible Mandibular canals" on radiographs, The prevalences of each type in order were 6.5 %, 18.8 %, 61.7 % and 13.0 %,

respectively.

2. The radiographic appearances of the mental foramen and the mandibular canal were classified into four types: A continuous type which showed continuity with the mental foramen through the mandibular canal. a separated type in which the mental foramen was distinctly separated from the mandibular canal, a diffused type in which the mental foramen with indistinct border and an unidentified type in which the mental foramen could not be identified on the radiographs. These prevalences in order were 25.3%, 21.7%, 22.4% and 27.6%, respectively.

3. The shape of the mandibular canals, when seen distinctly, were almost round and the average diameter of the mandibular canals was 2.0 ± 0.4 mm, encapsulated by thin cortical plates of 0.5 mm thickness

4. In regard of facio-lingual relation, the prevalence of pattern which the mandibular canal ran nearly to the lingual cortical plate. and turned abruptly towards the buccal side was 70% of observed samples.

5. According to superior-inferior relation, the prevalence in which mandibular canals had an angle with mandibular second molar was 39.0 % and that in which formed angle with mandibular third molar was 24.0 %.

6. As the Mandibular canal opened to the mental canal, the prevalence in which the mental foramen was located beneath the mandibular second premolar was the most commonly seen one (92.0%). Among them, the prevalence in which the mental foramen

located beneath the center of the mandibular second premolar, mesial of the mandibular second premolar and distal of the mandibular second premolar were 47.0%, 18.0 % and 27.0%, respectively.

7. On sectioned specimens, the distance between the inferior margin of the rental foramen and the inferior margin of the mandibular canal was 2.4 ± 0.7 mm.

8. On the sectioned specimens, the ratio of cortical bone to total area was the highest in the mandibular ascending ramus region, falling lower in the anterior region.

9. In terms of bony composition on the sectioned specimens, the mean cortical width of the facial cortical plate was increased toward the mandibular ramal region, and that of the lingual cortical plate was thicker in the anterior region than in the posterior regsion. On the other hand, the width of the inferior border was almost same in every region, but the thickest in the anterior region.

Taken all together, the mandibular canal of Korean adult runs along the lingual cortical plate of the mandibular body, encapsulated by the thin cortical plate. These results give us to speculate the mandibular malar region would be the best structure for the implantation. However, in most of the carte, the anterior border of the mandibular canal is located around the premolar region, and the mental foramen is opened nearly the second premolar. Therefore, it may be suggested that osteotomy has to be taken at the anterior place next to the canine region on genioplasty.
