Abstract
[한글]

위장관암의 진단에 있어서 종양표지자 측정의 임상적의의에 대한 연구가 국내외적으로

활발하다. 종양표지자는 첫째, 암의 발생위험군에서 암의 조기 진단을 위해 사용될수 있

으며 둘째, tumor burden을 예측하여 적절한 임상적 병기구분에 도움을 주고 셋째, 치료

효과를 판정하는데 이용되며 넷째, 암의 국소재발이나 원격전이를 밝히는데 사용될수 있

다. 최근 단일클론항체의 사용으로 새로운 위장관 종양관련항원(tumor-associated antige

n)을 확인하게 되었고, 이중 CA 19-9는 췌장암 및 담도암의 진단 및 치료효과 판정에 유

용한 검사로 여겨지고 있다. 그러나 위암은 한국인에서 가장 흔히 발견되는 암이나, 아직

이에 대한 종양표지자 연구는 거의 없는 실정이다. 최근 위장관암의 진단에 있어 새로운

종양표지자인 CA 72-4에 대한 연구가 진행중이며 특히 위암에 대한 임상적인 연구가 관

심을 모으고 있다. CA 72-4는 두 단일클론항체(CC49 및 B72.3)를 이용하여 혈청의 종양관

련당단백(tumor-associated glycoprotein, TAG72)을 측정하는 새로운 종양표지자이다.

이에 저자는 위장관 악성종양환자 273예, 위장관 양성질환환자 215에 및 정상대조군 93

예을 대상으로 새로운 종양표지자인 CA 72-4의 진단적의의을 중심으로 기존의 종양표지자

인 CA 19-9 및 CEA의 측정을 통해 각종 위장관암에서의 종양표지자의 역할을 확인하고자

본 연구를 실시하였으며 그 결과다음과 같은 성적을 얻었다

1) CA 72-4, CA 19-9 및 CEA의 평균치는 악성종양군에서 정상대조군이나 양성질환군보

다 통계학적으로 유의하게 높았다(p<0.05).2)

2) CA 72-4는 장기별 양성질환에서 95-100%의 높은 특이도를 보였으나 CA 19-9는 담도

질환에서 54%, CEA는 간장질환에서 63%의 낮은 특이도를 보였다.

3) CA 72-4는 위암에서 민감도가 497로, CEA(317) 및 CA 19-9(3l7) 보다 높았다.

4) CA 72-4는 췌장암 및 담도암에서 민감도가 각각 43% 및 44%로서 CA 19-9(각각 79%,

87%)보다 낮았으며, 대장암에서도 35%로 CEA(74%)보다 매우 낮았다.

5) 8예의 조기위암환자 전예에서 CA 72-4를 비롯한 CA 19-9 및 CEA가 모두 음성이었다.

6) 위암에서 임파절 전이 유무에 따른 CA 72-4의 차이는 얼었으나, 원격전이가 있는 환

자군에서 현저한 양성률을 보였다.

7) 위암의 병기별 CA 72-4의 분포는 4병기에서 83%로 1병기(7%), 2병기(8%) 및 3병기(1

3%)보다 현저히 높았다.

8) 근치적 위절재술을 시행한 5예의 위암환자에서 수술후 CA 72-4치가 정상상한치 이하

로 감소하였고, 항암약물요법에 반응을 보였던 4예의 진행성 위암환자에서도 약물치료 후

현저한 감소가 있었다.

9) CA 72-4는 전이성 대장암 및 췌장암에서 국소 및 국소진행성 대장암 및 췌장암에 비

해 현저히 높은 양성률을 보였다.

10) 위암환자에서 CA 72-4와 CA 19-9 또는 CA 72-4와 CEA사이의 상관관계는 없었다.

이상과 같이 CA 72-4는 췌장암 진단에 있어서 CA 19-9보다. 대장암 진단에 있어서는 CE

A보다 진단적 가치가 적었으나 위암에서는 가장 높은 예민도를 보였으며 4병기 이상의 진

행성 위암, 전이성 췌장암 및 전이성 대장암에서 현저히 높은 민감도를 보이는 바 CA 72-

4가 양성인 경우 치료방침을 결정하는데 신중한 고려가 필요할 것으로 생각된다. 또한 CA

72-4는 기존의 종양표지자에 비해 눈은 특이도를 보이는 바 CA 72-4가 양성인 경우 항상

악성종양의 가능성을 고려하여야 한다.







[영문]Tumor markers are related to the presence of malignant tumors.

Detection of tumor markers could offer an accessible method for screening rise groups in order to achieve an early diagnosis of cancers. to contribute to an adequate staging in expressing the tumor burden, and to help evaluate effects of therapy as well as to reveal metastases or local recurrences.

Recent use of monoclonal antibodies has led to the identification of new gastrointestinal tumor associated antigens. Among these, CA 19-9 is regaled as valuable assay for the diagnosis and for monitoring pancreatic cancer and biliary cancer.

But in spite of the high incidince of gastric cancer among the malignant tumors, especially in Korea, ideal tumor makes for gastric cancer have proved unreliable.

Recently, clinical trials using new tumor marker CA 72-4 are ongoing for gastric cancer.

In the following investigation, sera from patients with gastric cancer were analyzed for CA 19-9, CEA and new tumor-associated marker CA 72-4.

Material and method

Blood samples for the measurement of the tudor-associated markers CA 72-4, CA 19-9, and CEA were obtained after an overnight fast from581 patients in the following groups:1) 93 controls 2) 273 patients with malignant diseases 3) 215

patients with benign gastrointestinal diseases. Serum CA 19-9 and CEA were determined radioimmunologically with monoclonal antibodies. A cut-off value of 37 U/ml and 5 ng/ml were used, respectively. CA 72-4 antigen concentration was determined by a solid phase radioimmunometric assay using a CIS RIA kit ( CIS,

France). A cut-off value of 4 U/ml according to the mean±2SD was used. The results are summarized as fellows

1. The mean value of CA 72-4, CA 19-9 and CEA in malignant diseases were significantly higher than those of the benign digestive diseases(p<0.05).

2. CA 72-4 had high specificity ranged from 95% - 100% in benign disease groups but CA 19-9 (54%) had lower specificity in cholelithiasis anti CEA (63%) in liver cirrhosis.

In gastric carcinoma, the sensitivity of CA 72-4 (49%) was higher than that of CA 19-9 (31%) and CEA (31%).

4. The sensitivities of CA 72-4 was inferior to CA 19-9 in pancreatic cancer (43% versus 79%) and in bile duct cancer (44% versus 87%) and to CEA in colon cancer (35% versus 74%).

5. Eight of 45 rejected gastric carcinoma patients had 71 lesion(early gastric cancer), and CA 72-4, CA 19-9 and CEA were demonstrated all negative results for early gastric cancer.

6. In gastric carcinoma, there was no correlation between CA 72-4 positivity and occurence of lymph node metastasis but significant difference of CA 72-4 positivity was observed between those with distant metastasis and those without metastasis.

7. The positive ratings in stages Ⅰ,Ⅱ,Ⅲ, and IV were 7% (1 of 14 patients), 8% (1 of 12 patients), 13% (2 of 16 patients), and83% (24 of 29 patients) respectively and significant higher CA72-4 positivity was observed in stage IV gastric carcinoma.

8. Elevated levels of serum CA 72-4 decreased to below the cut-off value after radical gastric resection in five resectable gastric carcinoma patients and decreased also after chemotherapy in four chemo-responsive metastatic gastric carcinoma patients.

9. Comparing with local or locally advanced colon cancer and pancreatic cancer, high positivity of CA 72-4 was observed in metastatic colon cancer and pancreatic cancer.

10. No significant correlation was observed between CA 72-4 and CA 19-9 or between CA 72-4 and CEA in gastric carcinoma.

In conclusion, our data indicates that CA 72-4 is a reliable new tumor Marker of disease stagers and activity in gastric carcinoma. And CA 72-4 had a very high specificity in benign digestive diseases, so that elevated serum levels of CA 72-4

should always be taken seriours1y.
