한국인에게 적용시킨 통증평가법의 유용성에 대한 비교연구

Abstract

[한글]통증은 개인에 따라 느끼는 정도가 다르며 내적, 외적환경 및 심리적 상태등에 따라 다양하게 표현된다. 따라서 통증의 원인에 대한 평가가 아닌 통증 자체에 대한 평가는 환자의 주관적인 표현에 의존하게되며 이를 객관화 시키기 위한 여러종류의 통증평가법들이

개발연구되고 있다.

보다 적절한 통증평가법의 개발을 위한 작업으로, 본 저자는 1990년 4월 1일부터 1990년 8월 31일까지 본 연세대학교 의과대학 세브란스병원 재활의학과에 요통을 주소로 내원한 환자 143명을 대상으로, 외국문헌에서 흔히 사용하고있는 자가 통증평가법들의 임상적 유용성에 대한 비교연구를 실시하였다.

그 결과를 요약하면 다음과 같다.

1. 우리말로 도안된 각 평가도구들의 신뢰도는 매우 높았다.

2. 시각적상사척도, 단순서술척도, 구술적평정척도는 이해하고 기록하기에 쉽고 간단하며 통증의 강도를 잘 반영해주는 반면 통증의 다각적인 면을 고려하고 있지는 않았다.

3. McGill 통증질문서는 통증의 다각적인 면을 비교적 잘 묘사하고 있으나, 시간이 많이 걸리고 이해하기가 쉽지 않았다.

4. 여러종류의 통증평가법들중 통증장애지표는 통증으로 인한 일상적인 행동의 제한을 표현한다는 점에서, 재활의학과 영역에서의 보다 타당한 통증평가법으로 생각된다.

5. 임상에서 사용되고있는 여러종류의 자가 통증평가법들은 각 도구들의 특성에 따라 통증을 묘사하였지만, 임상적으로 평가가 필요한 통증의 지속시간, 하루중 빈도, 악화시키는 동작등에 대한 측면은 적절히 반영하지 못하였다.

이상의 결과로, 쉽고 간단하면서 임상적으로 필요한 통증의 여러 측면을 다각도에서 반영할 수 있는 새로운 형태의 통증 평가도구가 필요한 것으로 생각된다.

[영문]To develop a more adjustable method for subjective pain assessment through outpost study, we carried out the various pain measurement protocols on 143 patients who complained of low back pain.

The purpose of this study was to obtain information about the clinical value of various subjective pain assessment protocols which already had been used in the pale study.

A summary of results is as follows:

1. The reliability of each protocol for pain assessient which was translated to Korean was very high.

2. Simple Descriptive Scale, Visual Analogue Scale, and Verbal Rating Scale were simple and easy to perform, they reflected the intensity of pain but not the multidimensional aspects of pain.

3. The McGill Pain Questionnaire represented the multidimensional aspects of pain relatively well, but was time-consuming and difficult to understand.

4. Among all of these pain protocols, the Pain Disability Index was more valid in the fields of pehabilitation medicine stance it reflected altered behavior of daliy living due to pain.

5. Each pain protocol described the pain according to its individual characteristics. However, some variables such as duration or frequency of pain and aggravating motion could not be revealed.

According to the above results, a new form of subjective pain assessment protocol, which is simple and easy to perform and a multidimensional mirror of pain, is necessary.
