파킨슨병에서 자율신경계 증상에 대한 연구

파킨슨병은 운동완서, 근경직, 진전등을 특징으로 보이는 임상중후군으로, 특히 60세

이상의 노년인구에서 신경학적 장애를 일으키는 중요한 원인의 하나이다. 파킨슨병에서

특징적인 증상 이외에도 기립성 저혈압, 음위, 변비, 괄약근 조절이상, 피지루(seborrhea

), 발한이상, 유연증(Ptyalism)등의 자율신경계 증상도 나타난다. 몇몇 보고에서 이러한

증상들이 환자군에서 약 70-100%까지 나타난다고 하여, 파킨슨병에서 얼마나 자율신경계

의 이상이 흔하고 광범위하게 발생하고 있는 가를 보여준다. 이에 본 연구자는1990년 2월

부터 1990년 10월까지 연세대하교 의과대학 부속 세브란스 병원을 내원한 파킨슨병 환자

32명(남자 17명, 여자 15명)과 대조군 11명(남자4명, 여자 7명)을 대상으로 시행한 문진

과 비침습적인 자율신경계 검사방법을 통하여 다음과 같은 결론을 얻었다.

1. 자율신경계 증상을 호소하는 환자들은 전체 32명 중 29명(90.6%)에서 나타났고, 증

상들 가운데서 배뇨장애를 호소하는 환자가 15명(46.9%)으로 가장 많았고 다음으로 변비,

기립성 저혈압, 타액분비 과잉 등의 순으로 나타났다.

2, 안정상태에서 혈압과 심박동수는 환자군과 대조군에서 의미있는 차이를 보이지 않았

다.

3. 환자군과 대조군을 비교하면, pilocarpine 투여검사를 제외하고 시행한 모든 자율신

경계 검사 결과에서 유의한 차이가 관찰되었다.

4. 장애정도에 따른 변화에서, 장애정도가 심할수록 자세 변화 때의 30:15비율, Valsal

va비율과 등장성운동 후의 확장기혈압의 변화가 비례하여 감소되나, 유병기간에 따른 분

류에서는 모든 시행한 검사에서 증상이 5년 미만인 환자군과 5년 이상인 군 사이에 유의

한 차이는 없었다. 따라서 파킨슨병 환자의 자율신경 기능의 이상은 유병기간보다는 장애

정도와 더 밀접한 연관성이 있을 것으로 생각된다.

5. 장애정도로 분류하였을 때 제 I 기군에서 심호흡에 의한 심박동수의 변화와 등장성

운동 후의 확장기혈압 검사만이 대조군과 유의한 차이가 있는것으로 나타나, 파킨슨병 환

자의 자율신경계 이상을 측정하는데에 가장 감수성이 예민한 검사로 생각되었으며, 아울

러 파킨슨병에서의 자율신경계 이상은 중추자율신경계의 이상이 말초자율신경계의 이상에

우선하는 것으로 추정할 수 있었다.

[영문]

Parkinsonism is a syndrome that is characterized by bradykinesia, muscular

rigidity and tremor. It is a leading cause of neurologic disability in individuals

older than 60. The symptoms are variable. Among these symptoms, autonomic

dysfunctions such as orthostatic hypotension, ptyalism, impotence, bowel

dysfuntion, voiding difficulties, seborrhea and sweating disturbance are

encountered in about 70-lOO% of patients. In order to investigate the incidence and

characteristics of parkinsonian dysautonomia, we analyzed the data of subjective

autonomic symptoms and the results of various non-invasive autonomic function tests

in 32 idiopathic parkinsonian patients and 11 age-matched non-parkinsonian centrols

The results are as follows.

1. Among 32 parkinsonian patients, 29 patients (90.0%) had the symptoms of

autonomic dysfunction. The most common symptom was urinary disturbance which was

present in 15 patients (40.9%).

2. In resting state, the blood pressures and the pulse rates were not

significantly different between the patients and the controls.

3. The results of all performed autonomic function tests significantly different

between the patients and the controls.

4. The degree of parkinsonian dysautonomia was more significantly correlated to

the stage of parkinsonism than to the disease duration, which proposed that the

degeneration of autonomic nervous system in idiopathic parkinsonism was closely

correlated to those of nigrostriatal doparminergic system.

5. The sensitive autonomic suction tests distingishing mild (stage I)parkinsonian

patients from controls were E:I ratio and blood pressure changes during isometric

exercise, which showed the earlier autonomic changes took place in central rather

than peripheral autonomic nervous systems.
