지속성 외래 복막투석 (continuous ambulatory peritoneal dialysis:CAPD)이 혈액투석

에 비하여 여러가지 장점을 가지고 있으나 이를 시행하는데 여러 가지 장애요인이 있으

며, 특히 실패 원인 중 가장 중요한 것이 복막염이라는 것은 잘 알려져 있는 바이다. 이

에 복막염 발생에 관련이 있을 것으로 생각되는 성별, 연령, 거주지역, 결혼상태, 자발적

선택여부, 교육정도, 경제력, 직업의 유무, 용액교환시 청결여부, 일정한 투석액 교환장

소 유무와 투석기간, 투석액종류, 당뇨병, 고혈압, 고지혈증, 출구감염, BUN, 혈청 creat

inine 및 HBsAg을 위험인자로 설정하여 대상 환자 개개인에 대하여 조사한 후, 이들 환자

에서 복막염 발생빈도를 추적조사하여 복막염 발생에 관여하는 위험인자의 상대적인 중요

도를 확인하고자, 연세대학교 의과대학 세브란스병원에서 1990년 9월 1일 현재 지속성 외

래 복막투석을 시행중인 환자 237명 중 직접 설문지 조사 및 인체계측이 가능였고 6개월

이상 복막투석 치료를 지속하고 있는 환자 127명을 대상으로 하, 복막염의 연중 평균 발

생빈도와 지속성 외래 복막투석 시술 후 첫번째 복막염 발생까지의 기간 및 첫번째 복막

염 후 두번째 복막염 발생까지의 기간 및 이들에 영향을 주는 위험인자에 관한 조사를 하

여 다음과 같은 결과를 얻었다.

1. 대상 환자는 남자 53명, 여자 74명이었으며 연령은 17세에서 80세로 평균 45.8세이

었고, 지속성 외래 복막투석 기간은 6.1개월에서 80.4개월로 평균 28.7개월이었다.

2. 복막염의 연중 평균 발생빈도는 univariate analysis에 의한 분석결과 영양상태가

불량하고 직업이 없는 경우, 또한 Peritosol을 사용하고 교육정도가 낮은 환자에서 높았

으며(p < 0.05), multiple regression model에 의한 분석결과는 영양상태, 직업의 유무와

거주지가 의미있게 복막염 발생빈도에 영향을 주는 것으로 나타났다 (significant T < 0

.05).

3. 지속성 외래 복막투석 후 첫번째 복막염 발생까지의 기간에 대한 univariate analys

is에 의한 분석결과 영양상태가 불량한 환자에서 기간이 짧았으며(p < 0.05), Cox hazard

regression model에 의한 분석결과는 의의있는 위험인자를 발견할 수 없었다 (relatve r

isk < 0.5 또는 > 1.5).

4. 첫 번째 복막염 후 두번째 복막염 발생까지의 기간에 대한 univariate analysis에

의한 분석결과 직업이 없고 당뇨병이 있으며 여성인 경우에 각각 첫번째에서 두번째 복막

염 발생까지의 기간이 짧았으며(p < 0.05). Cox hazard regression model에 의한 분석결

과는 직업이 없고 당뇨병이 있으며 농촌에 거주하는 롼자에서 짧았다 (relative risk < 0

.5 또는 > 1.5).

5. 이상의 결과로, 복막염의 연중 평균 발생빈도에 의미있게 영향을 주는 위험인자는

양양상태, 직업의 유무, 투석액의 종류 및 거주지이며, 지속성 외래 복막투석 시술 후 첫

번째 복막염 발생까지의 기간에 영향을 주는 위험인자는 영양상태였으며, 첫번째 복막염

후 두번째 복막염 발생까지의 기간에 영향을주는 위험인자는 당뇨병, 직업의 유무, 거주

지로 밝혀져, 사회활동이 가능하도록 영양상태를 유지함으로써 복막염의 발생을 줄일 수

있으리라 생각된다.

Continuous ambulatory peritoneal dialysis (CAPD) has many advantages over

hemodialysis, but has certain inherent difficulties and Is well known its failure

due to peritonitis. The author studied CAPD patients of the YonSei University,

College of Medicine, Severance Hospital who were receiving CAPD up to Septemher 1,

1990. They were individually studied for relative risk of possible risk factors.

which are sex, age, place of domicile, status of Marriage, motivation, education

level, economic status. employment status, sterile technique exchanging a fluid

bag, place of exchanging a fluid bag, CAPD duration, type of dialysate, diabetes,

hypertension, hyperlipidemia, exit site infection, BUN, serum creatinine and HBsAg.

The follow-up study traced the frequency of peritonitis in a total of 237 subjects.

It was possible to study 127 patients who were subjected to a questionnaire and

nutritional score system and who were receiving at least 6 months of CAPD. Through

this study, material on the annual incidence of peritonitis, the interyal between

the insertion of CAPD and onset of first peritonitis, the interfal between the

onset of first peritonitis and second peritonitis and risk factors which influenced

the above subjects were obtained.

The results obtained were as follows :

1.The number of patients comprised of 53 men and 74 women. and the age

distribution was between 17 and 80 years with a mean of 45.8 years. The mean

duration of CAPD was 28.7 months with a range between 6.1 and 80.4 months.

2. The incidence of peritonitis was higher in patients with poor nutritional

status, no employment, and low education level according to univariate analysis (p

< 0.05). The results of the multipul regression model shewed that the significant

risk factors affecting the incidence of peritonitis were nutritional status,

employment status and place of domicile (relative risk < 0.5 or > 1.5).

3. The interval between the insertion of CAPD and the onset of first peritonitis

was shorter in poor nutritional patients according to univariate analysis. In those

subjects who were unemployed, had diabetes mellitus and who were women. the

interval between the onset of first peritonitis and second peritonitis was shorter

according to Cox regression model (p < 0.05).

4. The results of the Cox hazard regression model showed that the interval

between the onset of first peritonitis and second peritonitis was shorter in

patients without employnent, with diabetes mellitus, and those living in rural

areas (relative risk < 0.5 or > 1.5).

From the above results, it can be seen that risk factors significanly affecting

the incidence of peritonitis were nutritional status, employmet status. type of

dialysate and place of domicile. Significant risk factors affecting the interval

before the onset of first peritonitis uere not found, but the interval from the

first to second peritonitis was affected by significant risk fractors such as

diabetes mellitus, employment status and place of domicile. In conclusion,

maintenance of nutrition up to the level of normal physical activity can reduce the

incidence of peritonitis in the patients with CAPD.
