근육주사액 주입속도와 주사부위동통에 관한 연구 : 한국대학생을 대상으로

Abstract
[한글]

본 연구는 현재 임상에서 빈번히 사용되는 투약방법으로서 주로 간호사에 의하여 시행

되고 있는 근육주사에 따른 부작용 중 동통에 영향을 주는 요인을 규명하여, 올바른 근육

주사방법을 찾기 위한 시도의 하나로서, 근육주사시에는 주사용액을 느리게 주입하는 것

이 빠르게 주입하는 것 보다 동통이 덜 하리라는 일반적인 생각을 확인하기 위하여 시도

되었다.

본 연구의 대상은 대학 및 전문대학에 재학중인 19∼25세의 건강한자로서 본 연구에 참

여하기를 자원한 남자 30명. 여자 30명으로 이루어졌으며, 실험은 1988년 4월 20일부터 7

988년 5월 20일어 걸쳐 시행되었다.

본 연구의 방법은 대조군 사후측정법을 사용한 유사실험 연구로서0.9% 생리식염수 5ml

를 한 대상자의 양측복둔부위에 각각 5ml/초와 0.25ml/초의 속도로 2회 주사하여, 각 주

사 후 동통을 측정하였다.

본 연구에서 사용한 동통측정도구는 keel의 시각적상사척도(visu al analogue scale)과

Johnson 의 숫자척도를 혼합한 도구를 사용하였다.

연구의 가설검정은 paired t-test 와 그라프를 이용하여 비교 분석하였다

본 연구의 결과는 다음과 같다

본 연구의 가설 "근육 주사시 주사용액을 빠르게 주입한 후의 동통 정도는 느리게 주입

한 후의 동통점수와 차이가 있을 것이다"는 다음의 구체적 가설을 설정하여 검정하였다.

1. "근육주사시 주사용액을 빠르게 주입한 후의 시간별 동통점수는 주사용액을 느리게

주입한 후의 시간별 동통점수와 차이가 있을 것이다"는 주사 직후에 측정한 것만이 통계

적으로 유의한 차이를 보였는데, 이는 주사 맞을 때와 주사직후의 동통정도를 측정한 점

수로서 느리게 주입한 것이 더 높은 동통점수를 보였다.

그 이후 2분, 5분, 10분, 15분, 20분, 30분 후에 측정한 동통점수들은 모두 유의한 차

이를 보이지 않았다.

2. "근육주사시 주사용액을 빠르게 주입한 후 측정한 동통정도의 총합계점수는 느리게

주입한 후 측정한 동통정도의 총합계점수와 차이가 있을 것이다"는 기각되었다.(t = -.11

3. "근육주사시 주사용액을 빠르게 주입한 후의 동통완화양상은 느리게 주입한 후의 동

통완화양상과 차이가 있을 것이다."는 주사직후에만 느리게 주입한 측이 높은 점수를 나

타내었고, 2분 후에는 서로 같은 점수를 보였으며. 그 이후 시간경과에 따른 동통완화양

상은 서로 거의 유사하여 특별히 다른 양상은 보이지 않았다.

결론적으로 본 연구의 실험결과는 근육주사시 주사용적의 두 주입속도, 5ml/5초와 5ml/

20초에 따라 주사후 경험한 동통정도는 차이가 없는 것으로 나타났으며, 주사직후에 측정

한 동통 점수는 느리게 주입한 것이 더 높게 나타나, 근육주사시 주사용액은 느리게 주입

하는 것이 동통이 적으리라는 감각생리학적인 예상과 차이가 있었으며 일반적인 생각과도

일치하지 않았다. 이러한 결과는 사용한 주사용제가 조직에 자극을 주지 않으며 체액과

등장성인 생리 식염수였으므로 속도에 따라 차이를 보이지 않을 수 있으며, 또한 본 연구

에서 사용한 동통 측정도구가 대상자 자신의 주관적 동통 정도를 측정한 것이므로 빠른

주입시의 짧고 심하게 아픈것과 느린 주입시의 둔하고 오래 지속되는 아픔에 대한 동통정

도의 평가는 대상자 자신의 평가에 따른 것이었기 때문으로 사료된다.

앞으로의 연구에서 본 실험에서 사용한 주사용제, 용액의 양, 속도와는 다른 방법의 실

험을 동통에 근육주사시 주사용액의 주입속도가 동통에 미치는 영향이 더욱 명확히 규명

되어야 할 것이다.

This quasi-experimental study tested conventional concepts regarding the

relationship between the speed at which intra-muscular Injections were given and

the degree of patient pain arising from the injections.

It is generally accepted that intramuscular injections should be given slowly in

order to minimize local pain. in order to this notion, 60 subjects, aged 19 to 25

years, were given two shots -one fast (1ml/sec), one slow (0.25m/sec)- of 5mL of

0.9% NaCl solution in the right and left ventrogluteal site at an Interval of 30

minutes. Immediately following the injection and at the 2nd, 5th 10th, 15th, 20th,

and 30 minute the patients were asked to quantity of the degree of pain they felt,

by using the visual analogue scales with marked numbers of 0 to 10.

Three general hypotheses were tested.

1. The stores for pawn measured at each period will differ between fast and slow

injections.

2. The total scores for pain which measured after the injections will differ

between fast and slow injections.

3. The length of time recessing for local pain to disappea rafter injection will

differ according the rate of the injection.

The results of this study showed that the assumption

Contained in the first hypothesis was only valid immediately following the

injection, the slow injection showed core pain scores than fast injection.,. The

second hypothesis was deter-mined to be not valid, with regard to the third

hypothesis, there was no meaningful correlation between the rate at which the

intramuscular injections were given and the amount of time that elapsed before

local pain totally diminished. Immediate pain caused by slow infections was found

to be significantly higher than that caused by fast injections, however, after the

2 minute mark and for every mark after word patient pain was the same.

This study concludes that there is no correlation between the degree of pain

experimented by the volunteer subjects andt he rate of which intramuscular

injections are given.

This conclusion is at variance with the prevailing general view point. Therefore,

further test need to be conducted on using different injectables, volumes, and

speeds from this study.
