결핵환자 발견 및 관리를 위한 형광현미경 검사결과에 영향을 미치는 요인

Abstract
[한글]결핵 신환발견 및 치료관리에 이용되는 주요한 검사법으로 현미경검사와 배양검사가 있

다.

형광현미경 검사법은 Ziehl-Neelsen 염색에 비해 효율적인 방법이긴 하나 위양성률이

높다는 단점이 지적되고 있다.

어떤 실험방법의 민감도와 특이도는 연구대상에 따라 달라질 수 있을 것이다.

이에 이 연구에서는 1989년 4월부터 9월 사이에 서울지역 보건소를 통해 검사의뢰된 객

담가검물 13,783건을 신환발견군과 치료력이 있는 추구관리군으로 구분하여 배양검사를

기준으로 형광현미경 검사 결과에 영향을 미치는 요인을 분석하였다.

연구결과로 얻은 결론은 다음과 같다.

1. 신환발견을 위한 가검물의 양성율은 배양이 6.5%, 형광현미경검사가 1.4%, 추구관리

가검물의 배양이 10.1%, 형광현미경 검사가 10.8%로 나타나 진단을 위한 객담에서는 배

양이 형광현미경 검사보다 균양성률이 4.6배 더 높았지만 추구관리객담에서는 차이가 없

었다.

2. 형광현미경 검사의 민감도는 신환발견 가검물의 경우 15.5% 추구관리의 경우 74.9%

이었다. 특이도는 신환발견 가검물의 경우 99.6% 추구관리의 경우 96.4%로 나타나 객담이

채취된 대상에 따라 민감도, 특이도가 달랐다

3. 형광현미경 검사에서 1∼5개의 항산균이 관찰된 가검물은 10.5%만이 Ziehl-Neelsen

염색 양성이었으나, 11∼20개가 관찰된 가검물은 69.7%가, 41개 이상인 가검물은 전부가

양성이었다.

4. 형광현미경 검사에서 11∼40개의 항산균이 관찰되었으나 Ziehl-Neelsen염색 음성이

었던 가검물은 54.8%가 배양양성 결과를 보였다.

5. 집락이 20개 이하인 가검물의 형광현미경 검사는 신환발견군에서는 91.3%가 음성이

었고, 추구관리군에서는 44.4%가 음성이었다. +++ 이상인 가검물의 형광현미경 검사는 신

환관리군에서는 20.2%가, 추구관리군에서는 7.2%가 음성이었다.

6. 형광현미경 검사 양성인 가검물의 채취후 경과시간에 따른 배양결과를 보면 신환관

리군은 1∼3일에 검사시 90.9%가 양성이었으나, 4∼6일에는 72.1%가 양성이었다. 추구관

리군에 있어서는 1∼3일에 73.1%가 양성이었고, 4∼6일에는 70.9%가 양성이었다

이 연구에서 형광현미경 검사는 신환발견군 보다 추구관리군 가검물의 검사에 있어서

민감도가 높으며, 특이도는 두 군 모두에서 높고, 형광현미경 검사 양성이고 Ziehl-Neele

sn염색 음성인 결과가 모두 위양성은 아니며, 가검물 채취후 시일이 경과된 가검물의 검

사에서는 형광현미경 검사성적이 배양성적 보다 더 좋을 수 있다는 결론을 얻었다.

[영문]

Microscopic examination of smear and culture of sputum specimens are essential

for the diagnosis and follow up of tuberculosis. Fluorescence microscopy is

reported to be an efficinent method compared to Ziehl-Neelsen stain. However, false

positive result is a limitation. Sensitivity and specificity of a test may depend

on the study subjects. In this study, the results of fluorescence microscopy of

13,738 sputum specimens, collected during April through September, 1989, for case

finding and for follow up of treatment were analyzed. The following results were

obtained.

1. The positive rates were 6.5% by culture and 1.4% by fluorescence microscopy,

in the case finding specimens and 10.1% by culture and 10.8% by fluorescence

microscopy in the follow up specimens.

2. The sensitivities of fluorescence microscopy were 15.5% in the case finding

and 74.9% in the follow up specimens. The specificities were 99.6% in the case

finding and 96.4% in the follow up specimens.

3. Only 10.5% of specimens with 1-5 acid-fast bacilli on fluorescence microscopy

was Ziehl-Neelsen stain positive, but 69.7% of those with 11-20 and all of those

with over 40 were Ziehl-Neelsen stain positive.

4. Over fifty percent of specimens, with 11-40 acid-fast bacilli by fluorescence

microscopy but negative Ziehl-Neelsen stain, yielded M. tuberculosis by culture.

5. Fluorescence microscopy was negative in 91.3% of case finding and 44.4% of

follow up specimens with less than 21 colonies on media. In specimens with +++

colonies, 20% of case finding and 7.2% of follow up fluorescenc microscopy were

negative.

6. When the specimens were processed within 3 days, the culture positive rate was

90.9% in case finding and 73.1% in follow up specimens. The rate declined after 4

days.

It is concluded from this study that, the sensitivity of fluorescence microscopy

is higher in follow up specimens, the specificities are high in both specimens, not

all of the fluorescence microscopy positive but Ziehl-Neelsen stain negative

results are false positive, and higher positivity is obtained with fluorescence

microscopy than with culture in the examination of specimens which was delayed for

longer than a week.
