베체트 병의 구강궤양에 대한 국소인자의 영향에 관한 연구

Abstract
[한글]베체트 병의 최초 발생 주증상으로서 구강궤양이 가장 높은 빈도를 차지하며, 주증상의

평균 재발횟수, 주증상의 평균 유병기간에서도 구강궤양이 가장 높은 횟수 및 기간을 보

인다고 보고되어 있다.

따라서 구강내 국소인자에 의한 기계적 자극과 구강궤양의 재발과의 관련성 여부를 규

명하고자 본 연구에서는 1992년 1월에서부터 1992년 10월까지 세브란스 병원의 베체트 특

수진료실에서 베체트 병으로 진단 받은후 연세대학교 치과병원 구강진단과에 의뢰된 총 8

1명의 환자를 대상으로 설문조사, 구강검사, 치아석고모형, 임상병리검사를 통하여 구강

궤양과 국소인자와의 관련성을 조사하였다.

1. 분류아형은 가능형 55.55%, 용의형 19.76%, 불완전형 17.28%, 완전형 7.41% 순이었

고, 남녀비는 1:1.61이었으며, 평균 초발연령은 27.63년이었다.

2. 구강궤양의 호발부위는 혀 48.28%, 입술 23.15%, 협점막 20.69%, 구개 및 인두 4.43

%, 치은 3.45%의 순으로 혀에서의 발생률이 가장 높았다.

3. 임상병리검사 결과 혈청내 Herpes simplex virus (HSV)에 양성인 환자가 38.27%, C-

reactive protein (CRP)에 양성인 환자가 27.16%이었으며 Antistreptococcal titer (ASO)

가 양성인 환자가 12.34%, Rheumatoid factor (RF)가 양성인 환자가 9.87%, Antinuclear

antibody (ANA)가 양성인 환자가 3.7%이었다.

4. 구강궤양의 호발시기에 대한 설문조사 결과 피곤할 때 85.18%, 치솔의 외상 22.22%,

특정한 이유 없이 20.98%, 딱딱한 음식을 먹고나서 12.35%, 치아 및 보철물의 자극 13.5

8%의 순으로 피곤할 때가 가장 많이 호발한다고 응답을 하였다.

치아우식증 병력이 없는 환자가 24.69%, 치아우식증을 가지고 있는 환자가 23.46%, 치

아우식증 치료가 끝난 환자가 51.85%이고 편도염 병력을 가지고 있는 환자가 27.16%이었

다.

5. 혀의 전방부와 좌우측 협점막에 발생된 구강궤양과 각각의 부위에 형성된 국소인자

와는 서로 관련성이 있었다(p<0.01).

6. 입술과 혀의 좌우측 후방부에 발생된 구강궤양과 각각의 부위에 형성된 국소인자와

는 서로 관련성이 없었다.

이상에서 보는 바와 같이 구강내 치아에 의해 형성된 국소인자와 혀의 전방부와 좌우측

협점막에 발생된 구강궤양 및 이의 발생빈도와는 상호 관련성이 있다고 추측할 수 있어

베체트 병에서 국소인자가 구강 궤양의 재발과 치유 지연의 원인으로 작용한다고 보여 향

후 구강궤양의 치료시 전신적인 치료와 함께 이들 국소인자의 제거가 필요하리라 사료된

다.

[영문]

Of the many first-occurring major manifestations of Behcet's disease, the oral

ulcer is most frequently observed, shows the highest mean recurrence rate, and the

longest mean duration period. A study of the relationship between mechanical

irritation by local factors and the recurrence of oral ulcers was carried out to

see if there is a correlation. The subjects chosen for the study were 81 patients

who were referred to the Department of Oral Diagnosis at Yonsei University Dental

Hospital from the Behcet clinic of Severance Hospital after being diagnosed as

Behcet's disease patients.

1. By Shimizu's classification, possible type was the most numerous with 55.55%,

while suspected type was 19.76%; incomplete type, 17.28%; and complete type, 7.41%

were observed in that order. The male to female ratio was 1 to 1.61, and the

average age of onset was 27.63 years.

2. Oral ulcers developed most frequently on the tongue (48.28%), and lip

(23.15%), buccal mucosa (20.69%), palate\pharynx (4.43%), and gingiva (3.45%) also

showed ulceration.

3. 38.27% of the patients were HSV positive; 27.16% were CRP positive; 12.34%

were ASO positive: 9.87% were RF positive; and 3.7% were ANA positive.

According to the answers to the survey, fatigue (85.18%) was most frequently

associated with Behcet's disease. Trauma by tooth brushing(22.22%), un specified

reasons (20.98%), hard food chewing(12.35%), and irritation by dentition and/or

prosthesis (13.58%) were observed in association with Behcet's disease. 23.46%

suffered from dental caries. 51.85% were done with treatment of dental caries and

27.16% had a history of tonsilitis.

5. Oral ulcers on the anterior part of tongue and buccal mucosa were associated

with local irritants.

6. 0ral ulcers on the lip and posterior portion of tongue were not associated

with local irritants.

Local irritation by dentition and oral ulcer on the anterior part of tongue and

buccal mucosa was observed to coexit with one another; in other words, local

factors can act to cause of recurrence of an oral ulcer and to delay the healing

process in Behcet's disease. Therefore, elimination of local factors, along with

systemic therapy must be recommended when treating oral ulcers.
