전립선의 치골상부적 초음파조영 소견에 관한 고찰

전립선은 치골의 후부와 골반내 깊이 위치하고 그 해부학적구조상 X--선을 이용한 진단

은 어려운 것으로 알려졌다. (Watanabe등. 1975 ). 흔히 시생되는 전립선 촉지소견 ,혈액

생화학적소견, 방광요도경 검사, 방광요도촬영등은 전립선을 간접적으로 시사하는 검사들

이었다( Watanabe등, 1975 ). 최근에 초음파조영술의 개발로 명확한 전립선의 경제를 관

찰할 수 있으며 초음파의 투과에 의한 단면 영상을 봄으로써 여러 질환으로 초래피는 전

립선 내부의 변화도 구분이 가능하게 되었다 (Watanabe등, 1975 ; Resnick등, 1978 ) .

또한 초음파조영술은 방사선의 해독이 없으며 피검자에게 고통이 적고 어느 진단 방법보

다 정확한 장점이 있다 ( Henneberrr등, 1979). 저자는 기본 초음파기기외에 부속 기구가

불필요하며 시행이 매우 간편한 치골상부적 초음파조영술을 신제 임상에 적용하여 정상

과 각 질환에 따른 전립선의 영상을 비교 관찰하였으며 각 연령에 있어서 정산성인과 전

립선 크기를 측정하였고, 전립선비대증 환자의 수술전 전립선 크기의 측정은 수술방법의

선택에 중요한 바, 수술 전후의 전립선 크기를 비교하여 다음과 같은 결과를 얻었다.

1. 초음파조영상 정상성인에서의 전립선 모양은 방광의 기저부하에서 각이 둥근 삼각형

을 이루거나 타원형을 나타냈으며 좌우가 대칭이고 전립선의 경계가 명확하며 내부는 균

질한 점들로 분포되어 있었으며 전립선비대증 환자에서는 전립선이 난원형 혹은 원형을

이루며 전립선이 크고 방광의 기저부로 융기된 모습을 관찰할 수 있었으며 정상에서와 같

이 좌우 대칭이며 건계가 명확하고 내부는 역시 균질의 점으로 산포되어 있었다. 전립선

암의 경우, 초기의 암에서는 특이한 소견을 발견하지 못했으며 진행된 암은 경계가 불규

칙하고 좌우가 비대칭이며 내부도 비교적 불균질함을 발견하였다. 급성전립선염의 전립선

모양은 정상과 비슷하였으나 크기가 약간 증가되어 있었다.

2. 초음파조영 소견상 정상 성인의 전립선 크기는 21.30±4.80㎝**3 였으며 연령에 따

른 유의한 차이를 발견할 수 없었다.

3. 21명의 전립선비대증 환자들의 전립선은 초음파조영술로 측정한 결과 33.5 ∼ 61.

6 ㎝**3 의 크기에 해당하는 것이 11명(52%)으로 가장 많았으며 평균 57.4±17.2㎝**3를

나타냈다.

4.수술전 초음파조영술에 의한 전립선의 크기를 비교한 결과. 평균오차율은 77.58±8.1

5%였으며 상관계수 0.98 (P<0.005 )의 결과를 얻었다.

이상과 같은 결과로서 치골상부적 초음파조영술을 이용하여 전립선 질환의 감별에 도움

을 줄 수 있고 정확한 전립선의 크기를 측정할 수 있음을 알 수 있게 되었다.

There have been much limitations and errors in evaluating prostatic conditions by

traditional radiologic methods due to its location and anatomical strucrure.

However recent introduction and improvement of the ultrasonography have been

enabled us to visualize boundary of the prostate clearly and differentiate the

variable finding within the prostate. Transrectal or transurethral ultrasonography

of the prostate is popular nowadays, however we atraumatic to the patients, easy to

perform and its requires no adjustments or additions to basic ultrasound equipment.

In order to evaluate ultrasonogram of the prostate in patients with prostatic

diseases and normal adults and compare preoperative volume of the prostate on

ultrasonographic estimation with postoperative volum, 31 patients with prostatic

diseases and 40 normal adults under the age 60 were studied with suprapubic

ultrasonography.

The results obtained were as follows.

1. On ultrasonography picture of the prostate, normal adults showed symmetrical

or triangular or elliptical appearance and there were numerous fine homogenous

spots within the prostate. BPH patients showed symmetric, round or oval shapes and

its margine was smooth and numerous fine spots were seen within the prostate as

normal adults. The prostatic size enlarged and elevated to the bladder base.

Advanced prostatic cancer patients showed dyssummetric irregular appearance. The

prostate of acute prostatitis patients resembled normal prostate but prostatic size

enlarged.

2. The mean prostatic volum of normal adults on ultrasonographic estimation was

21.30±4.80 ㎝**3 and there were no differences of the prostatic volum between

ages.

3. The mean prostatic volum of 21 BPH patients on ultrasonographic estination was

46.2±17.2㎝**3 and majority patients were in 33.49∼61.56㎝**3.

4. Comparisonal studies between preoperative ultrasonographic prostatic volume

and resected prostatic volume showed correlation coefficient 0.98(p<0.005) and mean

error rate 17.58±8.1%. Most cases showed preoperative prostatic volume was larger

than postoperative volume which was probably due to inadequate removal of tissue

and surgical capsule.

As results of the above, the suprapubic ultrasonography was helpful in

differential diagnosis of the prostatic disease and estimation of the prostatic

size.
