타액선 종양에 대한 임상적 고찰

타액선의 종양은 주로 상피성 기원이지만, 어떤 간질요소(stromal element)에서도 발생

할 수 있다.

큰 타액선에는 양성 종양이 많고, 타액선의 크기가 작아질수록 악성 종양의 비율이 높

아진다. 발생빈도는 타 종양에 비하여 별로 높지 않아서 전체 종양의 1 ∼3%를 차지하고

있으며, 이하선에서 차지하는 비율이 가장 높다. 임상적인 진단은 물론 조직학적으로도

양성 종양과 악성 종양을 구분하는데 다소 난점이 있으며, 이하선의 경우에는 수술후 영

구적인 안면신경 마비가 올 수 있으므로 특별한 주의가 필요하다. 과거에는 이하선의 수

술은 no man's land라고 한적도 있으나, 지난 20년간 큰 진보가 있어 왔다.

다양성 선종(Pleomorphic adenoma)의 재발을 설명하기 위하여 일부에서는 종양 세포의

국부 침윤성을 주장하거나, 극히 작은 종양에서는 피막이 없을 수도 있기 때문이라고 했

다(Pensik,1957).

또한 종양 세포를 오염시킬 수 있는 불필요한 생검과 불완전한 절제가 높은 재발율을

가져올 수도 있다.

저자는 1965년부터 1977년까지 입원하여 치료받은 77예의 타액선 종양 환자의 임상기록

을 검토하고 원격조사를 시행하여 다음과 같은 결론을 얻었다.

1. 남녀 발생비율은 1 : 1.38로서 여자에서 약간 호발하였으며, 연령별 발생빈도는 30

대가 31.2%, 20대가 20.8%로서 20대와 30대가 전체의 반이 넘는 52%를 차지하였고, 악성

종양의 발생비율은 40대에서 50%, 50대에서 54%, 60대에서는 60%로서 연령이 증가할수록

악성종양의 발생도 증가함을 보여주었다.

2. 병력기간과 양성 및 악성 종양과의 관계는 양성 종양의 경우, 1년 이하의 병력기간

이 27%, 5년 이하가 73%였으며, 악성 종양에서는 1년 이하가 38%, 5년 이하가 84%로서 악

성 종양의 병력기간이 약간 짧았다.

3. 이하선 종양은 전 타액선 종양의 70.1%였고, 악하선은 22.1%,설하선 2.6% 기타 소

타액선이 5.2%를 차지하였다.

악성 종양은 전 종양의 41.6 %였는데, 타액선별로는 이하선의 43%,악하선의 29%,설하선

에서는 100%가 악성 종양이었고, 기타 소 타액선의 50%가 악성 종양으로서 타액선의 크기

가 작아 질수록 악성 종양이 차지하는 비율이 증가함을 볼 수 있었다.

4. 가장 호발한 종양은 다양성 선종으로서 전체의 54.5%를 차지하였고, 이하선에서는 5

5.6%에 달하였다.

악성 종양 중에서는 점액성 상피양암으로써 전체의 14.3 %를 차지하였다.

5. 가장 많이 사용했던 술식은 국부 및 광역 절제술로서 모든 술식의 42%를 차지하였으

며, 이하선의 경우에는 이하선의 표재엽 절제술로서 이하선 수술의 35.3%를 차지하였다.

6. 수술후 영구적인 안면신경 마비는 이하선 수술의 11.8%에서 발생했는데 대부분 종양

이 안면신경을 침윤하였던 예에서 발생하였다.

그러나 안면신경의 손상이나 회생시에 즉각적인 직접 문합은 3례에 블과하였고, 신경이

식은 한례도 없었음은 앞으로 많은 관심과 시도가 필요하다고 사료된다.

7. 평균 재발율은 37.7%였고, 양성 종양에서는 24%, 악성 종양의 경우에서는 43%의 재

발율을 보였으며, 다양성 선종의 재발율은 타 병원에서 한번 이상 수술받았던 환자를 포

함하여 23.8%에서 재발하였다.









[영문]

Author reviewed a total of 77 patients of salivary gland tumors who had been

admitted and treated in Yonsei Medical Center during the period from 1965 to 1977,

and evaluated their clinical pattern, histologic diagnosis, behavior and type of

treatment.

The following results were obtained.

1. Female was affected 1.38 times more than male, and the most frequent age group

wart 3rd and 4th decades that occupied more than half (52%) of all salivary gland

tumors.

2. The benign tumors had slightly longer duration than malignant tumors, and the

average overwhole duration was 4.8 years.

3. About 70% of all salivary gland tumors were located in the parotid gland, and

57% of the parotid gland tumors were benign.

Tumors of the submandibular gland occupied about 22% of all salivary gland tumors

and 71% of them were benign.

Tumers of the sublingual gland were about 2.6% of all salivary gland tumors and

all of them were malignant.

Minor salivary gland tumors occupied about 5.2% of all salivary gland tumors and

50% of them were benign.

4. The most common tumor of the salivary gland was pleomorphic adenoma (benign

mixed tumor) with the incidence of 54.5% of all salivary gland tumors and 55.6% of

all parotid g1and tumors.

The second most common tumor was mucoepidermoid carcinoma, which was 14.3% of all

salivary gland tumors.

5. The most frequent type of surgery performed for all salivary gland tumors was

either local or wide exeision comprising about 42% of all procedures, and for the

parotid lesions, superficial parotidectomy was the moat favoured operative method

comprising 35.3% of all parotid gland procedures. Postoperative irradiation was

performed in 16% of the patients and its resu1t was subtle to discern becauae of

unsatisfactory follow up.

6. Postoperative comp1ications included 6 cases of permanent facial nerve palsy

that was 11.8% of parotid gland tumor surgery, 2 cases of Frey syndrome and 2 cases

of salivary fistula.

7. The overwhole recurrence rate was 37.7%; 24% in benign tumors, 43% in

malignant tumors and 23.8% in pleomorphic adenoma including those were operated

more than one time before admission in this hospital previously.
