재활강화교육이 편마비환자의 자가간호 수행에 미치는 효과에 관한 실험적 연구

모든 환자나 장애자에게 필요한 재활간호는 전인간호의 목표를 달성하기 위해서도 매우

중요하다. 만일 재활간호가 제대로 수행되지 않는다면 환자는 기형(deformity)이 되거나

, 회복기간이 지연될 수 있기 때문에 이에 따른 경제적 부담과 인력의 소모를 초래할 뿐

만 아니라, 심리적인 문제를 야기시켜, 개인은 물론 가족, 사회적인 면에서도 많은 손실

을 가져오게 된다.

특히 뇌졸증으로 인한 편마비환자는 오랫동안 자가간호수행을 하지 못하면 의존적이 되

고 지적퇴행 및 우울증에 빠지게 되며, 가족은 계속 돌봐 주어야 하기 때문에 이로인한

문제점은 매우 크다고 볼 수 있다.

이러한 문제를 예방하기 위해서는 무엇보다 환자들이 가능한 한 빠른 시일내에 독립적

인 자가간호를 할 수 있도록 도와 주어야 하며, 따라서 이를 위한 효율적인 간호중재방법

의 개발이 요구되어 진다.

그러므로 본 연구는 이미 많은 연구에서 그 효과가 입증된 강화이론을 교육방법에 포함

시켜 재활강화교육이라는 간호중재방법을 고안하여 편마비환자의 자가간호수행능력에 미

치는 효과를 확인하는 임상 유사실험 연구를 시도하였다.

연구목적은 첫째, 재활강화교육의 효과를 검정하고 둘째, 환자의 재활자가간호이행과

자가간호수행능력의 회복에 영향을 미치는 요인을 분석 규명하는 것이었다.

연구방법은 1984년 7월 부터 9월 까지 K의료원 한방병원에 입원중인 편마비환자 61명을

대상으로 자가간호수행을 위한 재활교육을 실시한 후 실험군과 대조군을 무작위배정하여

실험군에게만 2회(제4일∼8일)(제11∼15일)의 강화를 주었으며 면접, 관찰, 보호자의 기

록을 통하여 자료를 수집하였다.

연구도구는 저자가 여러 문헌을 참고하여 개발한 자가간호수행능력척도(ADL 척도, 상지

의 조정능력측정도구), 재활자가간호이행 checklist, 회복 및 재활에 대한 의지, 가족의

지지, 환자-간호원관계 측정도구와 Wallston등의 건강조절위 척도(health locus of contr

ol), Zung의 우울척도를 사용하였으며, 재활강화교육을 실험(treatment)적 간호중재로 사

용하였다.

수집된 자료의 분석은 재활강화교육의 효과에 대한 가설을 검정하기 위하여 t-test와 P

earson correlation, 환자의 재활자가간호이행 및 자가간호수행능력의 회복에 영향을 주

는 요인을 규명하기 위하여 t-test ANOVA, Pearson correlation으로 분석하고, 주요예측

요인은 단계적중회귀분석(stepwise multiple regression)으로 확인하였으며, 그외 추가분

석은 chi-square test, Partial correlation, t-test로 분석하였다.

연구결과의 요약은 다음과 같다.

1. 재활강화교육의 효과분석

재활강화교육의 효과를 규명하기 위한 가설검정 결과는 다음과 같다.

제1가설: "재활강화교육을 받은 실험군이 강화를 받지 않은 대조군에 비하여 환자의 재

활자가간호이행정도가 높을 것이다"는 지지되었다. (t=5.06, df=59, p<.001)

제2가설 : "환자의 재활자가간호이행정도가 높을수록 자가간호수행능력의 회복정도는

높을 것이다"는 지지되었다. (r=.6816. p=.001)

제3가설 : "재활강화교육을 받은 실험군이 강화를 받지않은 대조군에 비하여 환자의 자

가간호수행능력의 화복정도가 높을 것이다"는 지지되었다. (t=7.45, df=59, p<.001)

2. 환자의 재활자가간호이행 및 자가간호수행능력회복에 영향을 미치는 제 요인

1) 환자의 재활자가간호이행에 영향을 미치는 제요인은 환자의 우울, 환자-간호원관계,

가족의 지지이며, 이와같은 요인들이 환자의 재활자가간호이행을 32.12% 설명할 수 있었

다.

2) 환자의 자가간호수행능력의 회복에 영향을 미치는 제요인중 가장 주요한 요인은 재

활강화교육, 재활자가간호이행, 강화교육전 자가간호수행정도, 발병일로 부터 물리요법

시작일까지의 기간, 치료비 부담자, 환자의 우울 순이며, 이와같은 예측요인이 환자의 자

가간호수행능력의 회복을 68.98% 설명할 수 있었다.

3. 추가분석

1) 제변수에 대한 두 집단간의 유사성검정을 한 결과 5% 유의수준에서 두 집단간에 유

의한 차이는 없었다.

2) 환자의 자가간호수행정도와 우울정도는 역상관 관계였다.(편상관계수 : partial cor

relation coefficient=.5067, p=.001)

3) 강화교육으로 환자-간호원관계도 긍정적인 변화를 가져올 수 있는 것으로 나타났다.

(실험전 t=-.42, df=59, p>.05 실험후 t=7.25, df=59, p<.001)

이상의 결과로 강화교육은 일회에 국한된 교육보다 교육의 효과가 훨씬 더 크기 때문에

장기간호를 요하는 환자에게는 더욱 바람직한 간호중재라고 할 수 있다. 잦은 강화는 환

자에게 많은 관심을 줄 수 있으므로 이행을 증진시키는 학습효과 뿐만 아니라, 심리적간

호도 겸할 수 있으며, 따라서 전인간호의 목적을 달성할 수 있다. 또한 환자-간호원관계

도 긍정적인 변화를 초래하기 때문에 다른 간호효과도 높일 수 있다고 본다.

Rehabilitation nursing required by all patient and disabled persons is quite

important for realizing the goal of comprehensive nursing-care. If rehabilitation

nursing is not appropriate, the patients would be deformed or recovery period would

be delayed. Therefore in-appropriate nursing results in not only economic burden

and less of manpower but also the psychological problems of the patients and their

families.

To prevent this problems, nurses should be help the patients perform self-care

activities as soon as possible. Thus this requires the development of effective

nursing intervention methods.

Therefore this study attempted a clinical quasi-experiment by designing nursing

intervention method as reinforcement Education for rehabilitation in order to

confirm the effect of the method under hemiplegia patient abilities in performing

self-care.

This study is aimed at testing the effect of reinforcement education for

rehabilitation and at analyzing and confirming the influences of compliance of

self-care for rehabilitation and recovery of performance abilities in self-care.

As a research methods this researcher devised 61 hemiplegia patients who

hospitalized at oriental medicine hospital of K University to experimental group

and control group by means of random assignment. And this researcher reinforced

experimental group only for two times (the 4th day - the 8th day)(the 11th day -

the 15th day) and collected data through interviews, observations and records of

families from July through September 1984.

The measurement tools developed and utilized by this researcher were self-care

scale, checklist on the compliance of self-care for rehabilitations patients' will

for recovery and rehabilitation, family support, relationship between patients and

nurses and reinforcement education for rehabilitation as experimental nursing

intervention. And this researcher adopted and used Wallston's health locus of

control scale, Zung's self-rating depression scale.

The data collected through above methods were analyzed by t-test, pearson

correlation to test a hypothesis on the effect of reinforcement education for

rehabilitation and they were also analyzed by t-test, ANOVA, pearson correlation,

stepwise multiple regression to clarify factors exerting influences on the

compliance of self-care for rehabilitation and recovery of performance abilities in

self-care. The other additional data were analyzed by chi-square test, partial

correlation, t-test.

The following is a summary of the results of this study.

1. The analysis of the effects of reinforcement education for rehabilitation.

1) The first hypothesis that the compliance of self-care for rehabilitation of

the experimental group given reinforcement education for rehabilitation would be

higher than the control group without reinforcement was supported.

2) The second hypothesis that the higher the compliance of self-care for

rehabilitation is the higher the degree of recovery in selfcare performance

abilities would be was supported.

3) The third hypothesis that the degree of recovery in self-care performance

abilities of the experimental groups given reinforcement education for

rehabilitation would be higher than the control group without reinforcement was

supported.

2. The analysis of factors exerting influences on the compliance of self-care for

rehabilitation and recovery in self-care performance abilities.

1) The factors were patients' depressions relationship between patients and

nurses, and family support and this factors can explain 32.l2% of the compliance of

self-care for rehabilitation.

2) The factors were reinforcement education for rehabilitation, compliance of

self-care for rehabilitation, decree of self-care performance abilities before

reinforcement education, a period between a day of attack and the first day of

physical therapy and those who pay medical fee, and this factors can explain 68.98%

of recovery in self-care performance abilities.

3. Additional analysis

1) There is no significant difference between experimental and control groups at

5% significance level as a result of a similarity test of two groups' varied

variables.

2) The relationship between the degree of self-care performance abilities and the

degree of depression was negatively correlated.

3) It was found that the reinforcement education can also positively change the

relationship between patients and nurses.

As a result of this study, educational effects of reinforcement education is more

conspicuous than a single educational program. Especialy this reinforcement

education can be a desireable for patients requiring long-term care. As frequently

given reinforcement programs can pay much attention to patients, the programs not

only promote efficiency of learning in compliance but also serve as psychological

nursing.

Therefore, the reinforcement education can achieve the goals of comprehensive

nursing care heightening the effects of other nursing because the reinforcement

education can result in positive change in the relationship between patients and

nurses.
