최근에 이르러 Graves씨병의 수술전처치로 종래까지로 사용하여 왔던 항갑상선제의 장기치료와 iodine 대신에, 단독으로 또는 이들과 병합하여 propranolol을 단기간 사용하는 경향이 많다. Propranolol을 술전처치에 이용할 경우 종래의 고식적인 방법에 비하여 술전처치의 기간이 단축되고 출혈량이 현저히 감소되는 잇점이 있음은 이미 여러 저자들이 밝힌바 있다. 그러나 Graves씨병의 안전한 수술을 위한 술전처치로서 propranolol만을 사용하여도 적절한지, 아니면 propranolol과 함께 항갑상선제 또는 iodine 제재와 병합 사용하는 것이 더 적절한지에 대해서는 아직 논란이 되고 있다.

이에 저자는 Graves치병의 술전처치에 propranolol을 이용하였을 경우의 안전성과 그 효과를 알아보기 위하여, 1980년 3월 1일부터 1982년 8월 31일까지, 만 2년 6개월간 연세의료원에서 propranolol 단독으로 또는 propranolol 과 PTU (propylthiouracil)를 병합하는 양식의 술전처치를 한후에 수술하였던 108예의 Graves씨병을 관찰한 결과 다음과 같은 결론을 얻었다.

1. 대상 108예의 성별, 연령별 빈도는 남자가 53예, 여자가 93예 였으며, 20대가 53%로 가장 많은 빈도를 보였고, 평균연령은 30세였다.

2. Propranolol 단독투여군은 22예로, 술전처치의 기간이 평균 11.5일 이었으며, propr anolol과 PTU의 병합투여군은 86예로, 술전처치의 기간이 평균 10.8일 이었다.

3. 수술방식은 양측엽아전절제가 82예, 일측엽전절제와 반대측엽아전절제를 병행하였던 예가 25예였다. 잔류 갑상선조직의 무게는 평균 4.5gm이었다.

4. 절제된 갑상선조직의 무게는 propranolol 단독투여군이 평균 58.5gm, 병합투여군이 평균 54.7gm이었으며, 수술중의혈량은 단독투여군이 평균 210.0㎖, 병합투여군이 평균 241.7㎖로 양군의 유의한 차이는 없었다.

5. 수술중의 맥박수는 단독투여군이 병합투여군보다 근소하게 빨랐으나, 양군 모두 수술중에 큰 변동이 없었으며, 퇴원시의 맥박수는 양군이 같았다.

6. 수술중의 평균최고체온은 양군에서 차이가 없었느나, 수술중에 또는 수술직후에 38.5℃의 열과 빈맥이 동반된 예는 단독투여군이 6예(27.2%)였고, 병합투여 군이 15예( 17.4%)였다.

7. 양군의 혈청T^^3 및 T^^4 는 수술후 3일 이내에 정상범주로 떨어졌으며, 수술후 3일까지는 단독투여군의 혈청T^^3 및 T^^4 가 병합투여 군보다 높았으나, 수술후 6일이후의 혈청T^^3 및 T^^4 는 양군사이에 유의한 차이가 없었다.

8. 단독투여 군과 병합투여군에서의 수술후 합병증은, 일시적 저칼슘증이 27.3%와 22.1%, 일시적 또는 지속적인 갑상선 기능 저하증이 13.6%와 18.6%, 재발이 4.5%와 3.5%였으며, 단독투여군의 예중에서 수술중에 고열을 보였던 1예가 수술후 제3일에 사망하였다.

이상과 같은 저자의 관찰에서, 양군 모두 짧은 시일내에 술전처치를 할 수 있었고, 갑상선의 무게나 출혈량 및 저칼슘증, 갑상선기능저하증과 재발등의 빈도에 있어서는 양군 사이에 유의한 차이는 없었으나, 수술중 또는 직후에 고열 또는 빈맥이 발생하였던 빈도는 단독투여군에서 현저히 높았다.

따라서 Graves씨병의 안전한 수술을 위한 술전처치로는 propranolol과 PTU의 병합단기 요법이 바람직한 것으로 사료된다.





[영문]

During 2 year and 6 month period, from March, 198O to August, 1982, prospective study was performed on 1O8 patients with Graves' disease, at Yonsei Medical Center.

Twenty trio patients preoperatively prepared with propranolol alone (group 1)were compared with 86 patients prepared with propranolol and propylthlouracil (group 11). The duration of preoperative preparation was an average 11.5 days in group 1,

and 1O.8 days in group 1l. Standardized thyroid resection, remaining 4 to 6 gm of thyroid gland, was employed. The pulse rate of group 1 was significantly faster than that of group 1l during operation, but there was no significant difference

between two groups on the day of discharge. The incidence of high fever or tachycardia during and immediately after operation was 27.2% in group 1, and 17.4% in group 11. This difference in the incidence of thyrotoxic sign was statistically significant. The incidence of postoperative transient hypocalcemia was 27.3% in

group 1, and 22.1% in group 11. Transient or prolonged hypothyroidisms occurred in 13.6% of group 1, and 18.6% of group 11. Recurrence of thyrotoxicosis developed in 4.5% of group 1, and 3.5% of group 11. One of the patients of group 1, who

developed high fever during operation, died on third day postoperatively. There was no significant difference in the average weught of the removed gland or the amount of blood less during the operation of the two groups.

In my experience, the use of propranolol provided rapid preparation of thyrotoxic patients for surgery, and there was no significant prevalence of complications such as hypocalcemia, hypothyroidism and recurrence in group 1, however the incidences

of high fever and tachycardia during and immediately after operation were significantly higher in group 1 than in group 11. Therefore, the use of preoperative propranolol in combination with propylthiouracil is recommended.
