모아관계에서 정상적인 조건의 어머니와 영아는 서로에게 효율적인 행동을 할 수 있는 기회가 보장된다. 그러나 출생직후에 고위험을 경험한 영아와 그의 어머니는 질병이라는 스트레스, 모아관계 형성기회의 결여, 모성역할 획득의 어려움 등의 여러가지 요인으로

인해 적응적인 모아 상호작용 양상을 이룩하는데 있어서 정상아 모아에 비해 불리한 입장에 있다고 볼 수 있다.

이에 본 연구는 출생직후에 미숙아나 질병으로 출생 7일 이내에 7일 이상 입원한 경험이 있고 조사일 현재 건강한 고위험아 모아와 출생직후부터 건강하였던 정상아 모아의 상호작용 양상이 차이가 있는지를 알아보기 위하여 모아 상호작용 행동의 빈도와 상호성 정

도를 측정하였다.

연구 대상자는 일 중소도시에 소재하는 1개 대학병원과 1개 종합병원, 1개 산부인과 의원에서 분만하였거나, 고위험으로 타병원에서 이송된 모아를 대상으로 선정된 정상아 모아 40쌍, 고위험아 모아 40쌍이었다.

자료 수집기간은 1992년 1월 3일부터 9월 30일까지였으며, 가정방문하여 비디오 카메라로 촬영한 다음, 2인의 훈련된 관찰자가 필름을 통해 관찰하였다.

연구 도구는 모아 상호작용 행동의 빈도를 측정하기 위해 본 연구자가 개발한 모아 상호작용 행동 목록과, 모아 상호작용 상호성을 측정하기 위해 하(1987)가 번역한 Walker(1982)의 모아의 놀이 상호작용 척도(Mother-Infant Play Interaction Scale)를 본 연구자

가 보완하여 이용하였다.

자료분석은 두 집단간의 일반사항의 유사성을 검정하기 위해 』x**2 검정을 하였고, 모아 상호작용 행동의 빈도와 상호성 정도를 알아보기 위하여 각 행동별 평균과 표준편차를 구하였다. 정상아 모아와 고위험아 모아의 상호작용 상의 차이를 t-검정하였다.

연구결과의 요약은 다음과 같다.

1. 모아 상호작용 행동의 빈도는, 어머니의 행등동 정상아 어머니의 경우 총 2293회, 고위험아 어머니의 경우 총 2286회, 전체 어머니의 경우 총 4579회였고, 범주별로는 두 집단과 전체 어머니의 경우 모두 발성이 가장 높았고, 다음으로 근육운동과 신체적 접촉, 영아 신체 돌보기, 놀이도구의 이용 순이었다.

전체 모아에서 모아 상호작동 행동의 빈도를 범주별로 보면 다음과 같다.

(1) 어머니의 발성범주에서 높은 빈도를 보인 내용은 유발성 단음, 반웅성 단음, 권유 흑은 지시순이었고, 낮은 빈도를 보인 내용은 노래, 설명순이었다.

(2) 근육운등과 신체적 접촉 범주에서 높은 빈도를 보인 접촉양식은 자세변화 시키기, 토닥거리기, 몸 흔들기 순이었고, 낮은 빈도를 보인 양식은 뽀뽀하기, 얼굴 맞대기, 세우기 흑은 걸음마 시키기 순이었다.

(3) 놀이도구의 이용범주에서 평균빈도는 0.86회로 매우 낮은 것으로 나타났으며, 소리나는 모빌의 이응빈도가 가장 높았고, 음악이 가장 낮았다.

(4) 영아신체 돌보기 범주에서 높은 빈도를 보인 행동은 포나 옷매무새 고치기, 목이나 얼굴닦기 순이었고, 낮은 빈도를 보인 양식은 분바르기, 트림시키기 순 이었다.

(5) 영아의 행동범주에서 높은 빈도를 보인 행동은 사지 흑은 머리 움직이기, 발성하기 순이었고, 낮은 빈도를 보인 행동은 딸꾹질하기, 재채기 흑은 기침하기 순이었다.

2. 모아 상호작용 상호성 점수는 정상아 모아의 경우 평균평점 3.65점, 고위험아 모아의 경우 3.34점, 전체 모아의 경우 평균평점 3.50점으로 보통수준(3점)보다 약간 높았다.

전체 모아에서 상호성이 높은 문항과 낮은 문항을 영역별로 살펴보면 다음과 같다.

(1) 어머니 행동영역에서 상호성이 높은 문항은 영아에게 하는 발성 Ⅱ(영아반응의 유발을 위한 발성의 회수), 시각적 상호작용, 영아에게 하는 발성Ⅱ(영아행동에 대한 반응성 발성의 회수)순 이었고, 낮은 행동은 영아를 돌보는 유형, 영아를 안는 유형, 영아의 행동에 대한 반응행동 순 이었다.

(2) 영아 행동영역에서 상호성이 가장 높은 문항은 지배적인 상태였고, 가장 낮은 문항은 시각적 상호작용이었다.

(3) 모아 동시적 행동영역에서는 정서의 등시성이 상호작용의 전반적인 양자간의 특성보다 높았다.

3. 모아 상호작용 행동의 빈도와 상호성간에는 유의한 상관관계가 있었다.

4. 정상아와 고위험출산아의 모아 상호작용 행동빈도의 범주별 차이는 다음과 같다.

(1) 발성빈도는 정상아 어머니에서 유의하게 높았고(t=2.82, p<.01), 내용별로 영아의 요구 흑은 반응의 확인(t=3.21, p<.01), 주위로 관심유도(t=3.47,p<.01), 영아의 발성흉내(t=2.58, p<.01), 웃음 흑은 미소(t=3.60, p<.01), 반응성 단음(t=2.04, p<.01)이 정상

아 어머니에서 유의하게 더 높았다.

(2) 근육운동과 신체적 접촉빈도는 고위험아 어머니에서 유의하게 더 높았고(t=-4.87, p<.01), 접촉양식별로는 발 흑은 다리를 만지거나 흔들기(t=-2.62,p<.01), 토닥거리기(t=-2.86, p<.71), 꼬집듯이 만지거나 때리기(t=-3.21, p<.01)가 고위험아 어머니에서 유의하게 더 높았다.

(3) 놀이도구의 이용빈도는 두 집단 사이에 유의한 차이를 보이지 않았다.

(4) 영아 신체 돌보기 빈도는 범주 전체에서는 두 집단 사이에 유의한 차이를 보이지 않았고, 유형별로는 목이나 얼굴 닦기(t=-3.16, p<.01)가 고위험아 어머니에서 유의하게 더 높았다.

(5) 영아의 행동빈도는 정상아에서 더 높았고(t=3.64, p<.01), 행동별로 사지 혹은 머리 움직이기(t=3,55, p〈.01), 어머니 얼굴보기(t=2.19, p<.05), 미소짓기(t=2.32, p<.05), 하품하기(t=3.11, p<.01)는 정상아에서 유의하게 더 높았으며, 보채기 흑은 울기(t=-3.50, p<.01)는 고위험아에서 유의하게 더 높았다.

5. 모아 상호작용 상호성은 정상아 모아가 유의하게(t=2,38,p<.05) 더 높았다. 영역별 두 집단의 평균평점의 차이는 다음과 같다.

(1) 어머니 행동영역에서는 정상아어머니가 유의하게 더 높았다(t=2.11,p<.05). 문항별로는 영아에 대한 근육운동 감각적 접촉의 특성(t=2.75,7.01), 시각적 상호작용(t=4.42, p<.01), 표정(t=4.89, p<.01), 영아에게 하는 발성Ⅲ(영아행동에 대한 반응성 발성의 회수)(t=2.42, p<.01), 애정표현(t=2.38,7.05)에서 정상아 어머니가 유의하게 더 높았다.

(2) 영아 행동영역에서는 정상아가 더 높았고(t=2.29, p<.01), 문항별로는 지배적인 기분과 정서(t=3.36,7.01)가 정상아에서 유의하게 더 높았다.

(3) 모아 동시적 행동영역에서는 두 집단간에 차이가 얼었고, 문항별로는 정서의 동시성(t=2.47, p<.05)이 정상아에서 유의하게 더 높았다.

이상의 연구결과를 종합해 보면, 영아의 상호작용 행동빈도는 정상아에서 더 높았으나 어머니의 행동빈도는 차이가 없는 것으로 나타났으므로, 고위험아 어머니는 상호작용 행동빈도가 낮은 영아에 대해 정상아 어머니보다 행동 수준이 더 높지 않았음을 알 수 있다.

또한 모아 상호작용 상호성은 어머니 행동영역과 영아의 행동영역에서 정상아 모아에서 더 높은 것으로 나타났으므로, 고위험아 모아는 상호작용과정에서 서로에게 나타내는 자극과 반응행동의 일치정도가 정상아 모아에 비해 더 낮은 것을 알 수 있다.

이러한 연구결과를 기반으로, 고위험아 모아의 적응적인 모아 상호작용 양상을 증진시킬 수 있는 간호중재를 개발하기 위한 연구를 제언한다.





[영문]

The importance of early experience for subsequent development is a virtually unchallenged assumption. of early experiences, the mother- infant relationship may be the most important, developed through continuous mother- infant interactions.

Numerous studies have demonstrated the dynamic process where by neonatal characteristics affect either concurrent or subsequent maternal & infant behaviors to produce a unique interactive relationship.

Postnatal illness or premature birth is one important factor that may contribute to the subsequent interactive difficulties of dyads of high-risk infants & their mothers.

The purpose of this study was to compare the mother-infant interaction in infants born under normal conditions with that in those born under high-risk conditions, to provide the basis for development of nursing interventions to help mothers achieve

an adaptive nether-infant relationship.

The survey through the non participant, structured observation method was designed.

80 mother-infant dyads from one university hospital, one general hospital, one OBGY clinics with pediatric department in one middle class city have been studied. Of the7, 40 dyads were high-risk group that had been hospitalized during more than

7 days starting within 7 days after birth due to illness or prematurity , 40 dyads were normal group that had been healthy since birth.

The mother-infant interaction was assessed at 4-6 weeks after birth(corrected age in the premature), by filming in unstructured free play situation in home and analyzed by two trained observers through taped films.

The data were collected from January 3 to September 30, 1992. The two instruments used for this study were Mother-Infant Interaction Behavior Repertoire( MIIBR ) developed by the researcher as behavior frequency measure, and Walker's

Mother-Infant Play Interaction Scale (MIPIS, 1982) modified by researcher as rating scale.

Analysis of data was done by using of x**2 -test, t-test, Pearson Product-moment Correlation Coefficient

The results of this study are summarized as follows:

1. The separate frequency distribution of each category in MIIBR was as for lows :

1) The total frequencies of paternal behaviors were 2293 in Bothers of normal infants, 2286 in mothers of high-risk infants.

The frequencies of four maternal behavior categories showed differences in descending order of vocalization category, kinesthetic exercise & physical contact category, physical care giving category, & use of play tool category in two groups & total mothers.

2) The total frequencies of maternal vocalization were 1520 in mothers of normal infants, 1145 in mothers of high-risk infants, and 2665 in total mothers. In maternal vocalization category of the two groups & total mothers, the contents with high frequencies were "single stimulative words", "dingle responsive words", "induction or direction," & those withlow frequencies were "song", "explanation"

3) The total frequencies of maternal kinesthetic exercise & physical contact were 605 in mothers of normal infants, 933 in mothers of high-risk infants, 1538 in total mothers. In maternal kinesthetic exercise & physical contact category of the two groups & total mothers, the styles with high frequencies were "changing position", "patting", "rocking", & those with low frequencies were "kissing", "facing".

4) The total frequencies of Maternal use of play tool were 69 in total mothres.

In maternal use of play tool category, the kind of the highest frequency was "musical mobile', that of the lowest 'music".

5) The total frequencies of maternal care giving were 124 in mothers of normal infants, 184 in mothers of high-risk infants, 308 in total mothers. In maternal physical care giving category, the types with high frequencies were "adjusting infant's bedding or clothes", "wiping the neck or the face", & those with low

frequencies were "powdering", "bubbling'.

6) The total frequencies of infant behaviors were 733 in normal infants, 536 in high-risk infants, 1269 in total infants. In infant behavior category, the behaviors with high frequencies were "moving extremities & head", "vocalizing", & those with low frequencies were "hiccoughing", "sneezing or coughing".

2. The score of reciprocity in mother-infant interaction was slightly high with mean score of 3.50(ranges 1-5).

Out of 18 items, the maternal behavior items with high scores were "vocalization style Ⅱ(stimulative)", "visual interaction", "vocalization style Ⅲ(responsive)", & those with low scores were "care giving style", "holding style" , "responsive style to infant behavior".

The infant behavior items with high score were "visual interaction" predominant infant mood/affect.

Of the dyadic behavior items, the score of "synchrony of affect" was higher than that of "overall dyadic quality of interaction"

3. There was a significant correlation between the mother-infant behavior frequency & reciprocity score,

4. There were partially signi ficant differences in the frequencies between the two groups in MIIBR.

In normal dyad group, the frequencies of maternal vocalization & infant behavior were higher than in high-risk dyad group.

In high-risk dyad group, the frequencies of maternal kinesthetic exercise & physical contact category were higher than in the normal dyad group.

There were no differences between the two groups in the use of play tool, physical care giving, total maternal behaviors repertoire frequencies.

5. The scores of mother-infant interaction reciprocity were significantly higher in the normal dyad group than in the high-risk dyad groups in material behavior category, infant behavior category, & total scale, and behavior items that were significantly higher than in high-risk dyad group were kinesthetic quality of interaction, maternal visual interaction, maternal expression of affect, maternal vocalization style Ⅲ(responsive),maternal expression of affect--quality of

contingency to infant, predominant infant mood/affect, synchrony of affect.

In summary, this study has shown that high-risk infants tend to be lower in their behavioral level & social reciprocity than their normal counterparts.

And, mothers of high-risk infants appear to be not more active in mother-infant interaction behavior frequencies & reciprocity than those of normal infants. thus to be not adaptive against low level of infant behavior.

On the basis of this study, the researcher thinks that it is necessary that nurses should identify the factors influencing these phenomena, and provide the mothers of high-risk infant with effective intervention to improve adaptive mother-infant interactions
