자궁 적출술후 나타나는 여러 증상들에 관하여 그간 상반된 여러보고가 있었으나 최근 에는 전향적 연구방법과 표준화된 척도를 이용한 정량적 분석으로 다음과 같은 결과를 보여주고 있다. 즉 수술후 여러 가지 임상 증세를 나타내는 여성은 수술전 이미 어느 정도의 정신과적 병태를 나타내며 정신적 질환의 발병조건의 정도가 수술전후에 있어 정상인 보다 높다는 것이다.

차제에 저자는 1986년 2월 1일부터 10월 31일 까지 연세대학교 의과대학 부속 세브란스병원에서 부인과적 적응력에 따라 자궁 적출술을 시행한 92명과 동 기간 중 자궁암 검진을 위해 외래로 내원한 환자중 신체검사상 정상으로 판단된 96명을 대상으로 대조군과 수술전 자궁 적출술 환자를 비교 관찰하고, 자궁 적출술 환자중 수술전후에 있어 우울과 불안정도, 신체적 증상 그리고 이에 관련된 요인들을 관찰하여 다음과 같은 결과를 얻었다.

1. 자궁 적출술군에서 수술전 자궁에 대한 이해정도는 자궁이 없어지면 기운이 없어진다-43명, 신체적인 이사이 올 것이다-40명, 부부생활에 나쁜 변화가 올 것이다-23명, 여성다움을 잃을 것이다-21명, 아기를 낳을 수 없다-17명, 기운이 난다-13명, 내자신이 쓸

모없는 사람이 될 것이다-9명, 남편의 애정에 좋은 영향을 줄 것이다-7명, 부부생활에 좋은 변화가 올 것이다-6명, 노이로제 같은 병이 생길 것이다-6명의 순으로 응답하였다. 또한 자궁이 없어지면 아기를 낳을 수 없다, 남편의 애정에 좋은 영향을 줄 것이다, 부부생

활에 좋은 혹은 나쁜 변화가 온다, 여성다움을 잃은 것이다 등은 연령이 적을수록 통계적으로 유의하게 많은 응답을 보였다.

2. 정상인과 수술전 자궁 적출술 환자를 비교시 SDS의 평균은 정상인이 39.98, 수술전 자궁 적출술 환자가 42.60으로 유의한 차이를 보였다. STAI S-A의 평균은 정상인 38.18, 수술전 자궁 적출술 환자 46.82로 유의한 차이를 보였다. STAI T-A의 평균은 정상인 39.8

1, 수술전 자궁 적출술 환자 40.90으로 유의한 차이가 없었다.

3. 자궁 적출술 환자에서 수술전후를 비교시 SDS의 평균은 수술전 42.60, 수술후 47.02로 유의한 증가를 나타냈다. STAI S-A의 평균은 수술전 46.82, 수술후 37.68로 유의한 감소를 나타냈다.

4. 자궁 적출술 환자에서 각 변인별 우울과 불안정도를 비교한 결과 연령, 교육정도, 종교, 경제수준, 성장지, 직업유무, 결혼상태, 가족력, 분만횟수, 난소보존 유무 그리고 빈혈유무 등에서 모두 유의한 상관관계가 없었다.

5. 정상인과 수술전 자궁 적출술 환자의 신체증상을 비교한 결과 수술전 자궁 적출술 환자에서 배뇨곤란, 질출혈, 체중감소 등이 유의한 증상으로 나타났으며 자궁 적출술 환자에서 수술전후를 비교시 수술전에는 두통, 심계항진, 백대하, 유방에 몽우리가 촉지됨,

유방증대 등의 증상이 유의하게 나타났으며 수술후에는 기력상실, 미각상실, 진땀이나 발한 복통, 배뇨곤란, 질출혈, 체중감소, 하복부 허전감, 수술부위의 동통 등의 증상이 유의하게 나타났다.





[영문]

There have been many clinical reports on the various symptoms that might emerge after hysterectomy. But the recent studies using prospective method and standardized scales tended to show the following results; A patient who showed definite clinical symptoms after hysterectomy might have already been present with

psychological morbidity and the level of psychological morbidity was much higher than general population both before and after hysterectomy.

The materials consisted of 92 patients who received hysterectomy at the department of Obstetrics and Gynetcology, Yonsei University College of Medicine from Feb. 1st to Oct. 31th, 1986 and 96 patients from general population who visited the out patient clinic of the department of Obstetrics and Gynecology,

Yonsei University College of Medicine for routine Papanicolau smear and confirmed healthy person by physical examination.

The severity of depression and anxiety and somatic symptoms were compared for the following 2 pairs of groups; first between the hysterectomized patients and the general population and second in each hysterectomized patient before and after

hysterectomy. the factors related to severity of depression and anxiety were also analysed.

The results obtained were as follows:

1. Preoperative understanding of how hysterecomy would affect the future physical and psychological well-beings were as follows: 43 patients answered that there would be loss of strength; 40 patients, physical changes; 23 patients, poor sexual

life; 21 patients, defeminization; 21 patients, loss of child bearing capability; 17 patients, bad effect to husband's affection; 13 patients, increased strength; 9 patients, feeling of worthlessness; 7 patients, increased affection from the

husband; 6 patients, enhanced sexual life and 6 patients, neurotic disorder.

And younger age group showed significantly many answers in loss of childbearing capability, good effect go husband's affection, enhanced or poor sexual life and loss of feminity.

2. The mean of SDS was 39.98 in the general population and 42.60 in the hysterectomized patients, showing statistically significant different between the 2 groups.

The mean of STAI S-A was 38.18 in the general population and 46.82 in the hysterectomized patients, again showing statistically significant difference between the 2 groups.

The mean of STAI T-A was 39.18 in the general population and 40.90 in the hysterectomized patients. Here statistically significant difference however were not seen.

3. The mean of SDS was 42.60 before hysterectomy and 47.02 after hysterectomy n the hysterectomized patients. This increase was statistically significant.

The mean of STAI S-A was 46.82 before hysterectomy and 37.68 after hysterectomy in hysterectomized patients. The decrease here was statistically significant.

4. Analysis of various individual characteristics of each patients-such as age, education, religion, economic status, growing place, occupation, marital status, family history of mental illness, parity and preservation of ovaries revealed no

correlation with the level of anxiety and depression.

5. when the somatic complaints of the general population and the hysterectomized patients were compared, dysuria, vaginal bleeding and weight loss were among the most commonly complained in the patients before hysterectomy.

Many patients before hysterectomy complained of headache, palpitation,leukorrhea, palpable breast mass and enlarged breast.

Common symptoms after hysterectomy were general weakness loss of taste, sweating, abdominal pain, dysuria, vaginal bleeding, weight loss, emptyness on lower abdomen and pains on operation site.
